HCAN part of a $4.5 million grant to address health-related social needs

HCAN is proud to be part of a $4.5 million grant to address health-related social needs in Greater Cincinnati. The grant was awarded to The Health Collaborative, in partnership with United Way of Greater Cincinnati, by the Center for Medicare and Medicaid Innovation’s Accountable Health Communities model. The Collaborative is one of 32 organizations receiving grants focused on reducing avoidable health care utilization in our community, impacting the cost of health care and improving health and quality of care.
