Health Care Access Now (HCAN) requests that its partnered Care Coordination Agencies (CCA) commit to the following:
- Ensure that Community Health Workers (CHWs) have been certified through an approved Ohio Board of Nursing program within the first year of employment.
- Deploy CHWs full- or part-time at a minimum of 0.5 Full Time Equivalency (FTE).
- Provide CHWs with supervision, participating in both supervisor and CHW training and continuing education.
- Maintain fidelity to the Pathways Community HUB Institute Model (PCH Model).
- CHWs should provide community-based care coordination to 30-40 active program participants living in Hamilton, Butler, Warren, or Clermont counties.
- Enroll program participants in the PCHI Model through in-person meetings to assess social needs and social risks.
- Conduct monthly visits to open and work through pathways.
- Complete health and social services pathways to address participant needs.
- Track progress through ongoing documentation in the PCHI Model data system.
- Provide each CHW and supervisor with secure internet-enabled devices (computer or tablet) and PCHI Model licenses.
- Ensure timely data entry into the PCHI Model data system, which enables reporting to funders.
- Respond to requests for documentation from the PCHI Model as need to maintain compliance.
CCA Onboarding Process
- Submit the application, which will be reviewed by HCAN staff.
- Selected applicants will be invited for an interview with key HCAN staff.
- If approved for selection, applicants will be notified and sent a PCHI Model contract/Business Associate Agreement.
- Talbert House IT will ensure all contracted CCAs’ devices (phones, tablets, desktops, laptops) are password protected, encrypted, and meet Health Insurance Portability and Accountability Act (HIPAA) standards.
- CHW(s) and Supervisor(s) attend PCHI Model training.
