Health Care Access Now and our incredible team of community health workers (CHW) are out in the community everyday working to reduce health disparities and create better outcomes for our area’s most vulnerable populations.

This evening celebrates the hard work and dedication of the CHWs, while raising awareness of the need in our community, and increasing access for those in need.
